Lafayette will be hosting a flu shot clinic on Thursday, Sept. 22 from 3-6 p.m. in Room 180.

Students need to fill out ‘Injection Consent Form’, and if under 18 students need a parent or guardian signature. All forms are found on Rockwood website here:

Shots are $28 for Trivalent vaccinations and $38 for Quadrivalent, and checks payable to Nursing Vaccination Specialists. Students can request a receipt to send to their insurance company for reimbursement.
